A new easy read discharge toolkit to support people with learning disabilities or mental health needs out of hospital has been launched.
The Discharge Toolkit and Discharge Planner are two new resources created from a project to help patients in specialist learning disability or mental health hospital services plan for leaving hospital.
The resources were produced by CHANGE with the support of NHS England, Local Government Association, and the Association of Directors of Adult Social Services.
Produced in easy read format, the Discharge Toolkit has lots of useful information:
- When will I be discharged?
- Where will I live?
- Getting help from an advocate.
- Who will support me?
- My rights.
- Community services.
- What if things don’t go to plan?
Also in easy read format, the Discharge Planner helps people work with others to make good plans for leaving hospital.
Individuals can use it to record information that they and the people who support them need to know about their hopes and choices as part of the plan.
The planner also helps keep track of meetings and who is doing what to make the plan happen.
The easy read discharge toolkit and planner are available for download from CHANGE's website.
